Application For a Death
Certificate

Same as applying birth certificate, the application must be submitted within 12 hours of the death the dead. Otherwise, the authority will impose a fee to the applicant.

Procedures For Applying Death Certificate
a) Before applying for death certificate (Sijil Kematian) to the national registration bureau (Sijil Kematian) ,families should requests the related parties to develop the proof of death first (Dokumen Bukti Kematian) .
b) If Death in the hospital, the hospital will process the related document.
If the deceased died of natural causes outside the hospital, health council commissioned from the National Registration unit or police will be responsible for issuance of proof of death .
If it is suicide, drown, traffic accident and etc (non-nature or the accident death, only the police is authorized to handle.
